How To Fill Out Your Medxpress 8500 Application

1. Before your appointment please be sure to go to MedXpress.faa.gov. You will then need to create a new account, make sure you do reset your password (different from the temporary password given).
2. In the top left side of the screen you will click on the 8500 form, fill out that form entirely and submit. Once submitted it will give you a confirmation number. Please bring in that confirmation for your appointment.
3. On the confirmation page there will be an “exam summary” button, click that. All the information you filled out will then be in the right format. Either SAVE or PRINT that form. You will need it the next time you need your  Medical Certificate reissued.
